Annual Survey

Thank you for another great year with our club! As we begin 2021 we want to get to know YOU better. Please take a moment to complete this short form. First, we want to know what you enjoy doing! Then, tell us what brought you to us! Finally, for the “profile” sections you may have a few from each profile but will probably notice that you are strongest in one specific profile. Don’t select anything that you aspire to be but only what is your natural tendency. For example, I am really great at working a crowded room and socializing but I am so uncomfortable when doing it and I would rather not. You would not select “social”. The goal is to match people to things and positions that they ENJOY and are naturally good at. Thanks for taking the time to help us get to know YOU better!
